CHOICE DAY
CHOICE DAY will take place on Friday, April 14, 2023
• You have other choices than suicide.
• Students who pay $3 will be allowed to participate in CHOICE DAY.
• Proceeds will be used to support Counseling Dept. programs.
• Students pick physical/healthy activities they want to participate in (For example, basketball, flag football, soccer, and relay race).
• We will also have face painting, a tie-dye station, and other games to play
• We will collect money from April 10- April 13 during homeroom/lunch (students can pay via MyPayments Plus, but we prefer cash for quick processing)
• The deadline to pay is April 13th at 12:30. NO EXCEPTIONS!
Complete THIS FORM in its entirety if you wish to participate in Choice Day. Once you have entered your information, don't forget to make your donation. Only students who donate $3 will be allowed to participate. Students can pay at school during homeroom/lunch or using MyPaymentsPlus (once you complete the survey, you will receive instructions).The deadline to make donations is Thursday, April 13th (you can make payments online until 12:30pm). NO EXCEPTIONS!
