The clinic is open daily from 9:00 AM until 4:00 PM. If you would like to schedule time to meet with our school nurse outside of these hours, please email Nurse Sam at samantha.stephens@cobbk12.org.

Preparing for next school year:

• Daily Rx medications needed during school hours, require a provider’s signature. Please print the Authorization to Give Medication form and take it with you to your appointment prior to the start of school in August. This is a 2-sided form, please let us reduce our paper use and print 2-sided.
• Emergency Asthma or Allergy medications can be kept in the clinic if the student is unable to carry with them. Please consider having your student always carry their emergency medications with them and having backups in the clinic. Updated action plans are needed each school year, along with the appropriate completed authorization forms for medications.

If your student needs a rescue inhaler and/or EpiPen, it must be provided by the parent/guardian. CCSD school do not stock emergency medications. We will call 9-1-1 for breathing/allergy emergencies.

• Diabetic students are required to provide updated DMMPs each school year. In addition to doctor’s orders, the appropriate authorization forms for medication administration, and management supplies need to be provided to the clinic at the start of the year and restocked as needed throughout the year.

CCSD schools do not stock any medications for students. Medication(s) must be provided by each student’s parent/guardian. Middle School students are permitted to have OTC medications as well as some prescription PRN medications on their person with the appropriate authorization form completed and turned into the nurse. Authorization forms do not transfer from year to year, a new form is required at the start of the school year.
