Clinic
The school clinic will be closed Thursday May 22nd until Tuesday July 29th for summer break. Emails or calls received during this will be addressed by July 30th. Nurse Sam will check email periodically throughout the summer and respond as needed.
PREPARING FOR NEXT SCHOOL YEAR:
CCSD schools do not stock any medications for students. Medication(s) must be provided by each student’s parent/guardian. Middle School students are permitted to have OTC medications as well as some prescription PRN medications on their person with the appropriate authorization form completed and turned into the nurse.
- Prescription medications, require a provider’s signature on the authorization form. Please print the Authorization to Give Medication form and take it with you to your appointment prior to the start of school in August. This is a 2-sided form, please help us reduce our paper use and print 2-sided. —Rx medications without a provider signature on the form and/or no corresponding action plan will not be accepted.
- Emergency Asthma or Allergy medicationscan be, and are encouraged to be, always carried by the student. This helps ensures their medication is close by them in the event of an emergency. Updated action plans are required each school year, along with the appropriate completed authorization forms. Please print the Authorization to Give Medication/ Carry Rx/Carry OTC Medication form and the action plan to take it with you to your next appointment prior to the start of school in August.
If your student needs a rescue inhaler and/or EpiPen, it must be provided by the parent/guardian. CCSD schools do not stock emergency medications. We will call 9-1-1 for breathing/allergy emergencies.
- Diabetic students updated DMMP paperwork is required each school year, along with the appropriate completed authorization forms for medications. Students should bring any management supplies and snacks to the clinic at the start of the year and restock as needed throughout the year.
Authorization forms do not transfer from year to year; a new form is required at the start of the school year. One form per medication.
Medications must be in original packaging with a readable label and dosing instructions. Medications cannot be expired. Liquid medications need to be unopened. Please send small or travel size containers.
