The Graduation Information Form is now open!  This form allows seniors to do the following: 

• NAME VERIFICATION - If the student's name is correct, DO NOT make any changes!  If the student's name is incorrect per the birth certificate, make the necessary changes.  If not already on file, a copy of the student's birth certificate may be requested. 
• PRONUNCIATION- If the student's name has a unique pronunciation, the student can list the correct pronunciation. 
• GRADUATION PLANS - The student indicates whether he or she will graduate in December or May.
• COMMENCEMENT PLANS - The student selects whether he or she will participate in the Commencement (graduation) Ceremony.
• CONTACT INFORMATION - The student enters a personal cell phone number and personal email address, NOT the CCSD email. 

To access the Graduation Information Form, students visit studentportal.cobbk12.org on their CCSD issued laptop.  Students will log in using their CCSD student email address, as well as their Office365 username and password.  Once you see the list of Apps, select Grad Form.  

Graduation Information Forms must be completed by Friday, November 22, 2024.
