Recommendations, Questionnaires, & Rating Scales

If a family requests the completing of a rating scale, form, questionnaire, or recommendation, we first need the request in writing from the parent/guardian. Teachers can also choose whether they want to complete the form or recommendation. Teachers can also choose whether they want to complete the form or recommendation. However, if they choose to complete it, the form or recommendation must be returned to the grade-level counselor. The counselor will send it directly back to the appropriate recipient. Please be sure to provide the point of return name and address or fax number. Should you choose to not put the request in writing, form JR-3 can be used. Your child's counselor can provide this for you. Thank you.
