Optional Parent Input Form
It is hard to believe that it is time to start thinking about and planning for the 2025-2026 school year. Each year, we solicit input from families regarding their child's classroom placement for the upcoming year. We request that families share their input with us via this form prior to classes being created. We utilize this information when creating classes. While we aim to meet the requests of every family, we carefully place each of our 700 students and we strive to accommodate requests to the best of our ability but sometimes are not able to do so. Creating classes is a collaborative process among all team members. As a school, we dedicate much time and attention to the placement process. A few components that are considered when creating classes are parent input, staff input, required student services, and class modules. Once class lists are posted and teachers are announced in July, changes will not be made. Please take the opportunity to share input with us now via the form below.
Below are a few guidelines to follow when completing the form:
- Please do not ask for a specific teacher by name or list any descriptors that indicate a particular teacher.
- Please do not request that your child be placed with another student, with the exception of twins.
- Submit a hard copy of your completed form to the attention of Mrs. Meador or Dr. Spooner by 3 pm on Thursday, April 3. We will not be accepting submission by email. Late submissions will not be accepted.
- If you have multiple children at Eastvalley, please fill out a separate form for each child.
You can send your form in an envelope with your child. You can drop it off at the front door during school hours or at the ASP desk until 6PM.
Forms will be accepted until 3PM on April 3.
