CCSD School Health Services

Medical Forms and Action Care Plans

Welcome to the Palmer School Clinic!

As a school nurse, I am proud to be a part of your child’s education. Success in school is directly related to the good health and emotional well-being of each student. Responsibility for the care of children lies primarily with parents. As a school nurse, I am here to assist and support parents' efforts to maintain good health in their children. School nurses do not diagnose; as nurses we assess and make recommendations based on that assessment. Care and guidance is provided according to Cobb County Schools' Guidelines.

Please visit the CCSD School Health Services page for detailed information and procedures regarding services provided by our school nurses.

Please

Please feel free to contact me with any questions or concerns, and I will do my best to respond by the end of the day.

Just a few reminders and “healthful” hints from your school nurse:

PLEASE KEEP YOUR CHILDREN AT HOME IF THEY DO NOT FEEL WELL!

BEFORE sending your child to school or on the bus, check your child every day for symptoms that include:

Fever or chills

Sore Throat

New cough

New severe/bad headaches

Fatigue

Shortness of breath, congestion or runny nose, trouble breathing

New Diarrhea

Muscle or body aches

Nausea or vomiting

Students must be symptom free/fever free without the use of of fever reducing medications for 24 hours before returning to school.

Does your student need medication at school?

Medical, Medication & Care Plans Forms can be found by clicking the link below.

All medications (prescription and non-prescription) MUST be sent in the original container, sealed, and labeled with the child’s name and RX. Please remember that the school health office cannot keep medications that are given at home on a daily basis in the event of a missed does (ex.  ADHD medications).

ALL medications must be hand-delivered to the nurse’s office/clinic by the parent/guardian. Children are NOT allowed to transport prescribed medication to the school. Students may carry some medications determined by the grade and maturity level of the child along with signed permission forms. Please direct any questions to the school nurse and school district policy JGCD-1.

Allergy, Asthma, and Seizure Action Care Plans can also be found at this link. These forms expire one year from the date signed. If your student is going to carry their inhaler or epi-pen a Care Plan action form and authorization to carry form is still needed to be kept in the clinic.

Food Allergies and Outside Food

Many students have food allergies. It is imperative that parents notify the school nurse and the child’s teacher of any food allergies as soon as possible. To keep students safe, we ask that parents Please DO NOT bring in outside food, other than lunches and health snacks for their own child’s consumption. A food allergy can turn into a potentially life-threatening situation. Please have your doctor complete an Allergy Action Plan form for the school to share with your student’s teacher and to notify the cafeteria manager.
