All medication must be brought to the clinic in the original container by parent/adult and a signed authorization form completed before a student may receive medication.

•    My student has asthma:

Students who require medication at school need an authorization form and asthma action plan signed by the physician

•    My student has allergies:

Students who require an epi-pen at school need an authorization form and an allergy emergency action plan signed by the physician.
