PLEASE NOTE: PER CCSD POLICY, ALL MEDICATION THAT IS KEPT IN THE CLINIC MUST BE DROPPED OFF DIRECTLY BY PARENT/GAURDIAN WITH THE SCHOOL NURSE AT THE SCHOOL. PLEASE DO NOT SEND IN PRESCRIPTION MEDICATION WITH YOUR CHILD, AS NURSE VANESSA WILL NOT BE ABLE TO ACCEPT IT FROM THEM. PLEASE CONTACT NURSE VANESSA TO SCHEDULE A TIME TO BRING IN YOUR CHILD’S MEDICATION: VANESSA.MOULTHROP@COBBK12.ORG.

NOTE FROM NURSE VANESSA:
I know this is a lot of information, but please read through it carefully. I like to provide parents with all the pertinent information upfront, before you come, so there aren’t any surprises and wasted trips back-and-forth to the clinic. Thank you!  

WHAT’S NEW?: Cobb County School District, is now requiring ALL prescription (Rx) medications to have the Healthcare Provider's Signature on the Authorization to Give Medication form (form JGCD-2). In order for the school nurse to administer prescription medications to students, the parent/guardian must provide the form signed by both the Healthcare Provider and the parent/guardian. The medication administration instructions on the signed JGCD-2 form must match the instructions on the prescription (Rx) label. 

Nurse Vanessa can only accept medications in the clinic that meet the following requirements:

Prescription (Rx) Medication Requirements: 

  • All Rx medications should be in their original container (i.e., no loose pills in a Ziplock baggie)
  • The Authorization to Give Medication form (form JGCD-2) must be signed by the parent/guardian AND the provider for ALL Rx medication. (Over-the-Counter medications DO NOT require a provider’s signature)
  • The Rx label instructions should match the instructions written on the JGCD-2 form, signed by the provider
  • All Rx medication must have the following clearly and legibly printed on the label:
    • First and Last Name of student
    • Name of Medication
    • Dose
    • Time(s) to administer.   [PLEASE NOTE: please make sure that the time(s) you want your child to receive their medication at school is consistent with the Rx label and the administration instructions provided on the JGCD-2 form, signed by the provider. Legally, Nurse Vanessa can only administer the medication consistent with the time(s) listed on the Rx label].
  • Please check the expiration date, as Nurse Vanessa cannot accept expired medication.
  • The pill description on the label should match the pills inside the bottle.
  • Please bring in no more than a 30-day supply of daily medication.

Over-the-Counter (OTC) Medication Requirements: 

  • All medications should be in their original container (i.e., no loose pills in a Ziplock baggie)
  • The JGCD-2 form only requires a parent/guardian signature for OTC medication. OTC medication does not require a provider’s signature.
  • Please check the expiration date, as Nurse Vanessa cannot accept expired medication.

ADD/ADHD Medications:
If you would like your child to take ADD/ADHD medication at school, please know that, per CCSD protocol, those medications are only allowed to be given on a daily basis. We cannot keep ADD/ADHD medication to be taken on an as-needed basis. 

Carrying Emergency Medication
In middle school, students are allowed and encouraged to carry their emergency medication at school (i.e. EpiPens, Auvi-Q, Benadryl, antihistamines). If you would like your student to carry their emergency meds at school, please complete the appropriate form: Authorization to Carry Over-The-Counter Medication or Authorization to Carry Rx Medication.  Bring it in, along with the medication that your child plans to carry, so Nurse Vanessa can verify and sign off on it. Please make sure that inhalers and EpiPens/Auvi-Qs are in the original box with the prescription label on it. If that is too bulky, you can remove the prescription label from the box and adhere it directly to the inhaler or EpiPen. This helps us know who the medication belongs to in case it gets misplaced, and it verifies that your student has a current prescription from their doctor.

 Carrying OTC Medication
Students may also carry the following OTC medication in middle school: Tylenol/acetaminophen, Motrin/Advil/ibuprofen, Midol, aspirin, antacids, cough drops and oral antihistamines. If you would like your student to carry these medications, please complete the Authorization to Carry Over-The-Counter Medication.  Bring it in, along with the medication that your child plans to carry, so Nurse Vanessa can verify and sign off on it. 

Please Note:

  • Each medication requires a separate form, so if you want your student to carry more than one medication, please fill out a form for each medication.
  • Hard copies of all the forms are available in the clinic
