Our Mission
When medical or mental health needs prevent a student from attending school for a limited period of time, Hospital Homebound coordinates a temporary plan for continued academic participation.

What is Hospital/Homebound?
Hospital/Homebound (HHB) is an academic service designed to provide continuity between the classroom and home or hospital for students whose medical needs, either physical or psychiatric, do not allow them to attend school for a limited period of time (Georgia State Board of Education Rule 160-4-2-.31 HHB services). HHB services are not intended to supplant regular school service and are by design temporary.
Where do parents get the HHB application?
School counselors are the point of contact for the HHB process. Please speak with your student’s counselor about the need for services and to obtain an application.
What if my child only requires periodic absences due to his or her illness?
Hospital/Homebound supports intermittent services for students diagnosed with chronic health conditions causing absences of 3 consecutive days per occurrence for a minimum of 10 days per year.
Can my child's pediatrician complete the Medical Referral Forms for a psychiatric/emotional diagnosis?
Following direction from the State of Georgia, we only accept diagnoses for psychiatric or emotional illness from a Georgia Board certified, licensed psychiatrist. Medical Referral forms submitted by psychologists, counselors, therapists or pediatricians are not eligible for HHB services.
What is an Educational Services Plan (ESP)?
The Educational Services Plan, or ESP, is developed in a school team meeting to determine what a student’s HHB services will look like. This meeting occurs after HHB eligibility, and may include some of the following individuals: HHB instructor, parents, teachers, school counselor, administrator, school psychologist, social worker, or school nurse. The purpose of the meeting is to clarify information pertaining to your child’s medical needs, instructional delivery, modifications, duration of services and other important HHB considerations.
What is the parent's role in the HHB process?
Parents play a key role in the HHB process. Aside from having the treating physician or psychiatrist complete the Medical Referral Forms, parents are responsible for seeing that the student has a quiet, distraction-free location to work with an HHB instructor. Parents are to monitor their child’s academic progress weekly as students are expected to complete assignments when the instructor is not present. Parents (or a trusted adult 21 years or older) must remain in the home for the duration of the instructional visit. Consult the Guidelines for Hospital/Homebound Instruction for more information.
Can my child participate in extracurricular activities while eligible for HHB?
CCSD does not allow students who receive HHB services to participate in extracurricular activities, including travelling or competitive sports teams.
How will my child take the EOC/Milestones tests while on HHB?
Students who are physically able to take the tests at school will do so. For those unable to test in person, the school will coordinate with Hospital Homebound to determine an appropriate testing plan.
What happens when my child no longer needs HHB services?
Once a student is medically released to return to school, HHB services are considered complete. HHB services are also considered complete when a student returns to school for any portion of the school day.
